The patient experiences symptoms such as chronic cough, shortness of breath, and frequent respiratory infections. COLLABORATION PROCESS The patient’s primary care nurse, Sarah, collaborates remotely with a team of healthcare professionals, including a pulmonologist, a respiratory therapist, and a pharmacist, to provide evidence-based care for COPD.
